If you host your own and have access to iptables, you can use the
geo-ip module.  You can poll the Maxmind db free once a month.  Or you
could more properly do it at your border router if your network was
designed as such.  There are certain parts of the Internet that you
don't ever want to exchange packets with.  I don't have a solution for
shared hosting where you don't control the actual server, I don't play
in that world any longer.
