Twilio theoretically works better in the USA, Clickatel everywhere else. 

Once confirmed, Clickatel lets you send using a cell phone number you have. Short codes and such can be a nightmare unto themselves.

Depending on the volume, an old cell phone with a USB cable works pretty darn well, and I have sample perl code that we use in emergencies when we have to.
